If your sponsor or employer requires a tax invoice issued in the organisation’s name, they must complete this form and provide a valid purchase order number.

If your sponsor or employer is unable to supply the required information for a tax invoice, your fees statement/invoice will be issued in your name (the student). In this case, you are responsible for ensuring that your fees are paid by the due date
