Addendum forms: Faculty of Health and Environmental Sciences programmes

AUT has legal obligations in relation to students engaging with the public during their studies. These apply to programmes which include placements, particularly those educating students in disciplines covered by the Health Practitioners Competence Assurance Act (2003) and/or the Vulnerable Children’s Act (2014), as well as students whose learning may involve practice in which they may potentially come into contact with children.

Additional documents for Faculty of Health and Environmental Sciences programmes

All applicants to the Faculty of Health and Environmental Sciences (FHES) who have been offered a provisional or conditional place for these programmes are required to submit the following additional documents:

  • International Applicant Addendum
  • FHES Postgraduate Addendum
  • FHES Undergraduate Addendum
  • NZ Police Vetting Request & Consent Form The NZ police vetting request and consent forms must be received by the faculty's compliance office within three months of  being signed and dated. This is a requirement of the Police Vetting Service.
  • Certified copies (see below how to certify your documents) of one Group A and one Group B Identity document (one of which must be a photo ID)

    Group A ID documents include

    • Birth certificate
    • NZ citizenship certificate
    • NZ refugee travel document
    Group B ID documents include
    • NZ drivers license
    • 18+ card
    • Community Services Card
    • Employee identification card
    • Student ID card
    • Teachers registration
    • Electoral roll record
    • International driving permit
  • Referees Nomination Form - ONLY required for International students enrolled in BHSc (Midwifery), BHSc (Nursing), BHSc (Occupational Therapy), BHSc in Oral Health, BHSc in Paramedicine, BHSc (Physiotherapy), BHSc in Podiatry, Diploma in Paramedicine and all students in Master in Psychotherapy

How to certify your documents

If you’re in Auckland, New Zealand

  1. Bring the original document to AUT and show it to either the Student Hub or departmental administration staff
  2. A copy will be made and signed by the staff member as being a true and accurate copy

OR

  1. Take the original document and a photocopy of it to a Justice of the Peace, Notary Public, Member of Parliament, a barrister or solicitor, Registrar or Deputy Registrar of the Courts
  2. Please note that the title/position of the person certifying the documents must be stated on the document and be legible
  3. Send in the certified photocopied documents with your application form. Remember, all documents must be certified and if you don’t do this you can’t be enrolled - please don’t send originals

If you’re overseas

Take the original document and a photocopy of it to an AUT registered agent or your country's equivalent to a Justice of the Peace, Notary Public, Member of Parliament, judge or any person authorised by the law of your country to administer an oath.

Note: Don’t send original documents (passports, birth certificates) as they will not be returned.

The University may suspend or terminate your enrolment if any falsified documents are discovered during the application or enrolment process, or during your study.

  • Submit these in person or in hard copy
  • Don’t upload these documents to Arion as they will not be accepted in electronic format

Submit your documents to AUT Student Hub

  • North Campus: Level 2, AS Building (through the food hall)
  • City Campus: WA atrium, level 2 (ground entry)
  • South Campus: MB Building
