Please complete this form if you are in managed or self-isolation; especially if you have been instructed by the Ministry of Health (MOH), Healthline or a general practitioner (doctor) to have a COVID-19 test because you have been in contact with a person who is a confirmed case, and are now isolating.
If you have a confirmed case of COVID-19, please make sure you tell MOH that you are an AUT student. The MOH will then contact the University on the next steps if necessary.
