COVID-19: Isolation form for AUT students

AUT is working to ensure students, staff and the general public are safe. We also want to ensure you are supported if you are impacted by COVID-19 and are isolating.

Please complete this form if you are in managed or self-isolation; especially if you have been instructed by the Ministry of Health (MOH), Healthline or a general practitioner (doctor) to have a COVID-19 test because you have been in contact with a person who is a confirmed case, and are now isolating.

If you have a confirmed case of COVID-19, please make sure you tell MOH that you are an AUT student. The MOH will then contact the University on the next steps if necessary.

For AUT updates visit our frequently asked questions on Student Hub Online. If you have any urgent concerns, contact Healthline on 0800 358 5453 (New Zealand) or +64 9 358 5453 (overseas).

For all other queries, please contact the Student Hub on or phone 0800 288 864 (New Zealand) or +64 9 921 9779 (overseas).
